Admissions Procedures:
1. The following completed enrollment forms are to be returned to the Academy office:
- Enrollment Application Information
- Parental Financial Contract
- Parental Commitment Form
- Family Information Forms
- Principal/Counselor Recommendation Form
Applicants must turn in the following forms to the Academy to be a part of their student file at the time they bring the Enrollment Application forms:
Current original immunization form (expiration date must be current through end of school year – June, 2010.)
- Copy of birth certificate
- Copy of social security card
- Copy of health insurance card
- Copy of custody papers, if applicable
- Academic records: report cards for current year and previous two years
- Achievement test scores for past two years
2. Every student pays a $450.00 enrollment fee. The $150.00 application fee portion is due when applying, along with a $25.00 entrance test fee. The $300.00 balance is due upon receipt of a letter of acceptance from CHCA. Students registering after March 15 must pay the full $450.00 enrollment fee and $25.00 entrance test fee at the time of application.
Test date/interview time will be given upon receipt of all paperwork.
Note: Public registration for the 2009-10 school year beginning date:
Tuesday, January 20, 2009.
Registration forms will be sent home and posted online on Monday, January 5th, 2009.
Registration dates for currently enrolled students :
Tuesday, January 6th – Thursday, January 15th, 2009
Cottage Hill Baptist Church members, and siblings of enrolled students, may begin registering on Friday, January 16th , 2009.
Public registration begins on Tuesday, January 20th, 2009.
**NOTE After March 15, the $150.00 application fee and the $300.00 balance of the enrollment fee are due at the time of application, along with the $25.00 entrance test fee for new applicants. All fees are non-refundable
